No, British citizens are no longer automatically entitled to free healthcare in Spain. However, there are several ways to get health insurance coverage in Spain as a British citizen.

However due to Brexit EHIC is no longer available. For those who have the EHIC card, the expiry date will be the last day of free health insurance in Europe.
